Amsler Grid

The Amsler grid is a useful tool to detect vision problems resulting from damage to the macula (the central part of the retina).

Report any irregularity to your eye care professional immediately.

The grid does not replace having your macula tested by an eye care professional, particularly if you are over 50 yrs old or have any risk factors for macular degeneration.

How to do it

  1. Cover one eye, then focus on the dot in the centre.
  2. Do any of the lines look wavy, blurred or distorted?
  3. Are there any missing areas or dark areas in the grid?
  4. Don't forget to test
    both eyes.

Macular Degeneration New Zealand Professional Friend Programme Terms and Conditions

By submitting the Macular Degeneration New Zealand (MDNZ) Professional Friend Programme (Programme) Application you agree to the following Terms and Conditions.

Membership of the Programme is not an endorsement or recommendation by MDNZ of the Member and should not be represented as such by any individual practice.


1. Professional Friend Programme

Programme membership is conferred upon an individual (Programme Member). If the Programme Member operates in more than one practice the benefits of the Programme will extend to the other practices. Programme membership is not transferrable to another individual.

Programme benefits and cost are outlined in the invitation letter. These may be amended by MDNZ from time to time.

Membership of the Programme is considered effective from the date payment is received for 1 year. Membership may be renewed annually by payment of the annual membership fee in accordance with item 6.

 

2. Practice display materials

Programme Members will receive a certificate of membership that can be displayed within each practice location under the Programme membership. A Programme Member is permitted to display the Programme certificate (or other materials as supplied by MDNZ from time to time) in relation to their membership for the duration their membership remains current.

If Programme Membership is terminated (see item 8), the Programme Member must remove all Programme materials from display and cease to represent themself as a Programme Member.

Programme Members must not use any trademarks owned by MDNZ other than by displaying the Programme certificate and any other materials supplied by MDNZ in accordance with item 2.


3. Web directory

MDNZ will have a national database of members from 2015. Programme Members will have listing(s) highlighted as being a Member of MDNZ’s Professional Friend Programme. MDNZ will endeavour to ensure that Member information displayed on the web-based national directory remains current but accepts no responsibility for the accuracy of the information provided by the Programme Member. Responsibility for the accuracy of the information provided to MDNZ lies with the Programme Member.

Any changes required to a Member’s information as displayed in the web-based directory must be requested in writing to MDNZ. MDNZ will amend the listing as soon as practicable.


4. Resource orders

Programme Members can order MDNZ publications and resources free of any postage and handling charges.
Free postage and handling on resource orders applies to a maximum of one order per calendar month for each practice location. Orders must be placed on the MDNZ Professional Friend Programme Resource Order Form that will be provided and should include the Programme membership number. Orders placed by any other means will incur the standard postage and handling fee.


5. Use of information

Information provided to MDNZ by the Programme Member will not be used for commercial purposes but MDNZ will publish contact details in its online web-based directory. Except via its web-based directory, MDNZ will not sell or intentionally make available to any other organisation information provided by the Programme Member.

Should MDNZ receive a request from a Programme Member to be removed from the web-based directory or to not receive communication from MDNZ, MDNZ will comply with this request at the earliest possible convenience.


6. Payment

The annual membership fee for the Programme may be paid via cheque, electronic funds transfer or credit card. For payment via credit card, the fee will automatically be charged to the credit card number provided at the time of application at each annual renewal unless instructed otherwise by the Programme Member or if the Programme membership is terminated. For payment via other means, the Programme Member will be contacted 1 month prior to the annual Programme renewal date to arrange payment of the annual membership fee. Payment must be made within the time period specified in the notice.


7. Dispute resolution

Both MDNZ and the Programme Member will respond to any queries, issues or complaints submitted in writing by either party within 5 working days. Each party will endeavour to resolve any issues.


8. Termination of Programme membership

The Programme Member may terminate programme membership at any time by writing to MDNZ.
MDNZ may terminate Programme membership if the Member fails to adhere to the Programme Terms and Conditions and there has been no acceptable outcome to any dispute resolution (see item 7), or if the Programme membership fees are unpaid.

MDNZ will not provide any Programme membership fee refund irrespective of the reasons for termination.
